to Meant 2B Loved Pet Rescue Society (Society).

I understand that, if approved, giving said pet to the above named Society means the Society becomes sole owner of said pet. I will make no attempt to reclaim said pet. I fully grant the Society the right to give said pet any medical treatment deemed necessary, and to place said pet into a good adoptive home.

Should a veterinarian determine said pet is too ill to be treated, I give the Society the right to humanely euthanize said pet.

I release the Society from all liability concerning said pet and acknowledge that the Society will make every attempt to provide care and find a permanent home for said pet.
